The bleeding is excessive either in amount or duration or both which is diagnosed by passage of big clots, increased number of sanitary pad usage, or soilage of clothes despite pads. rnrnLevel of hemoglobin can give an idea of magnitude of disease. Bleeding could be due to increased surface area of uterus like fibroids, hormonal imbalance, ovarian tumor or even IUCD in the uterus. When there is no discernible cause and irregular menstruation occurs it is called as dysfunctional uterine bleeding. This is related to excess of estrogen hormone. This diagnosis is made when biopsy of inner uterus is not abnormal. The treatment is of appropriate cause and drugs like haemostatics, hormones and antifibrinolytics are used as given below: rnrnDosage rnrnTo immediately stop the bleeding Norethisterane 5 to 15 mg/day is given. Withdrawal bleeding occurs after 3 to 5 days of stoppage.rnrnLater oral pills are given from 5 to 25th day for at least 3 cycles (periods) or. rnrnDanazol 200 to 400 mg/day in 4 divided doses for 3 months.rnrnProstaglandins inhibitors like Mafenamic acid 150 to 600mg/daily.rnrnAntifibrinolytic drugs such as Tranexamic acid 1g four times a day.rnrnOther coagulants used are Ethamsylate, Bioflavonoids and Rutin.rnrnSurgery-Dilatation and curettage (D & C) for removal of hyperplastic endometrium. rnrnEndometrial ablation and resection - Laser ablation, roller ball ablation and transcervical resection (TCRE) are some of the methods. Overall success rate is 70 to 80 per cent.rnrnScanty Menstruation rnrnThis is of two types: 1) Oligomenorrhoea: When menstrual bleeding is more than 35 days apart with regular frequency, and 2) Hypomenorrhoea: When bleeding is unduly scanty and lasts less than 2 days.rnrnScanty menstruation can be due to many reasons. It can be during adolescence (at the onset of menstruation), preceding menopause, and in obese women. It can be due to stressful conditions like stress of examination, anorexia nervosa (a psychological illness where patients have no desire to eat), and psychogenic. Sometimes due to endocrinal problems like thyroid dysfunction polycystic ovarian diseases and hyperproteinaemia some women may have less/scanty menstruation. In tuberculosis also this condition is observed. The primary cause is taken care of and appropriate drugs used. Cyclic hormonal treatment is needed to restore normal menstruation.
